2017-2018 PIAA Sports Physical Pre-Participation Information Packet & InstructionsPIAA CIPPE Sections 1-6All students interested in trying-out for a District-sponsored athletic team are required to complete a Pennsylvania Interscholastic Athletic Association (PIAA) Comprehensive Initial Pre-Participation Physical Evaluation (CIPPE).A CIPPE may be performed no earlier than June 1st; and, regardless of when performed during the school year, remains effective only until the next May 31st.As a service to its students, East Pennsboro Area School District will offer physical examinations provided by Enola Family Practice for $15, at East Pennsboro Area High School, during the summer. Note: No sports physicals will be given at the school before the winter or spring seasons. Families may opt to go to their primary care physician for the examination or Pinnacle Health Sports Medicine.In preparation for the physical examination, student-athletes MUST arrive with PIAA CIPPE Sections 5-6. Section 5 must be completed by the student-athlete and parent/guardian, and Sections 5-6 must be made available to the examining doctor. Without these forms, a physical examination cannot be provided.The EPASD Athletic Department is utilizing an online registration process for all student-athletes. Physical examination forms, medical history forms, and participation fees should be submitted to the Athletic Office prior to the enrollment deadline. All other information and athlete certification forms will be gathered through Rank One Sport, which is ready at this time. This form is used to re-certify an athlete for a sport. Section 7 should be used for the WINTER and SPRING seasons only "IF" an athlete has already completed a physical exam in an earlier season. Forms and fees should be dropped off to the Athletic Office.PIAA CIPPE Section 8*PIAA Physical Exam RE-CERTIFICATION Form (Re-Certification By Licensed Physician of Medicine). Forms and fees should be dropped off to the Athletic Office.All Physical Exams, ImPACT Test results, Signed Forms, and Fees are due by the "Enrollment Date" listed below; however, it is encouraged that these aspects are submitted immediately following the acquisition of a physical exam dated after June 1st. 3/12/2018*Football players must have all of their enrollment forms and fees submitted to the athletic office by 5:00 p.m., 7/31/2017, due to the heat acclimation period. The enrollment deadline for all other sports will be extended to 8/7/2017 due to the unexpected delay regarding the Rank One set-up.
